This is an order pad from a shoe repair shop (Reliable Shoe Shop) located in New Castle, PA.  The pad is complete, starting with order #1 and continuing through order #50.  There is carbon paper inserted between pages of each order; the shop kept the "carbon copy" and the customer was given the original, which was presented when returning to pick up the repaired shoes.


The decade printed on the order pad forms is shown as "193_", making the pad more than 90 years old.

This rare, complete order pad measures 6" x 3.5" x 0.5"; it weighs 3.3 ounces.  It has been stored in a smoke-free, climate-controlled home.
