The choke lever shaft is slightly bent. Carb looks to be complete. It is dirty & will need to be thoroughly cleaned and will need to be rebuilt. The float bowl edged have some small pieces broken off, but this does not affect its use. 

The number on this carb is Keihin Carb # 569A.
