Miles Ames Contrast Light Model: 4520.  This item will need a new cord before final use.  This powers on and appears to be in good working condition.  It also appears to be in good physical condition but the cord has some chafing and it arced once while I was moving it around.  Again, cord should be replaced.  Please view pictures for physical condition.
